Health Action Plans

The Health Action Plan says what services and support a person with a learning disability needs to have a healthy life.

It also contains a list of people who may be able to help.

The person with a learning disability usually keeps a copy of the Plan, which may have pictures and easy words, or be on tape.

All people with a learning disability should be offered a Health Action Plan.
