Person Centred Plans - Planning your life

What it is and how it can help you?

Person centred planning is about making sure that you are at the centre when planning your life.

By being at the centre of planning it means that you are the most important person

Question

The plan will have information about:

  • What is important to you
  • What you want to do with your life
  • What you will do to work towards getting what you want from your life
  • How friends, family, carers and others who may work with you can support you

How do you get a Person Centred Plan?

You need to sort out your ideas about what you want to do with your life. You can have help to do this.

The person who helps you make your plan will record what the important people in your life have to say.

They will help you put all the information together into a plan.

The plan can be on paper, DVD, CDRom, CD or on more than one of these.

Your plan will be about you. Everyone’s plan will be different.

The plan should be kept up to date and looked at regularly to make sure that it is working properly for you.
