To help you plan for housing and support you will need to find out what support is available.

Housing

If you already see a worker from the Learning Disability Service you can ask them for information about housing and support.

Contacts

If you don't see a worker from the Learning Disability Service, you will need to make a referral to the Learning Disability Service.

A referral means that you are asking for help and support.

Information

Who Do I Contact?

You can contact the Learning Disability Services and explain that you would like to speak to a member of staff because you want help.

They will ask you for some basic information and will be able to tell you what will happen next.

If you do not want to make the referral yourself, if you agree, someone can make the referral for you.

This could be:

  • a relative
  • a carer
  • a friend
  • an advocate

You can also speak to your doctor or other professional and they can make the referral for you.

Friends

A worker will meet with you and work out with you what need help and support you need. This is called an assessment.

The assessment is free but there may be some charges to pay for some of the support you get. This will depend on what money you have.

It will only take into account your savings and income. It will not take into account the savings or income of anybody that you may live with, such as your parents or a carer.

  • Sometimes equipment in the home can help people to be independent.
  • Some people will have an occupational therapy assessment to decide if they need any equipment.
  • Occupational therapists also work with people to help be more independent.
Equipment

Carers might also want to ask for a joint or separate carer assessment to help them think about the support that they may need now and in the future.

This can be a joint assessment or a separate assessment.
